What causes Low Back Spasm? Easy tips to get out of it

A research analyzing low back spasms through the lens of Ayurveda (calling it Katigraha) confirms that the most effective relief comes from a simple, two-step approach: calming the nervous system and relaxing the muscle tissue.

If you feel like a tightening, pulling or twitching of the muscle in your lower back, it could be a painful back condition known as a low back spasm. In such a situation, the sufferer often gets confused about what to do and what not. Should they exercise or go on bed rest?

In this article, I’m going to clear all your confusion regarding this. But to understand it better, we first need to know what a low back spasm is and what causes it.

What Causes a Low Back Spasm?

In a normal resting condition, our muscles remain in a relaxed state when there is no tension in the muscle fibres. But when we work or move our body, the concerned muscle gets into action, resulting in increased tension in the muscle fibre.

Muscle spasm is a condition in which the tension in the muscle fibre does not relax even if the body comes to rest. The muscle remains in the taut, contracted state, which produces a great deal of pain and discomfort.

But what’s actually happening inside the muscle?

According to a critical analysis published in the Journal of Technology, a lower back spasm isn’t just a simple cramp:

  1. It’s a complex process where nerve endings in the muscle (nociceptors) become highly sensitized1
  2. Strong stimuli like trauma or overloading, as well as inflammatory chemicals in the body (like bradykinin and prostaglandins), can trigger these nerves. 
  3. This leads to a cycle of pain and contraction.
  4. Furthermore, a drop in pH levels (increased acidity) in the muscle tissue, often caused by poor blood flow (ischemia), is a key factor in activating pain signals.
  5. This explains why the area can feel so tender and why movement is so painful1.

This is why it is very important to relax the spasmodic muscle to cure this pain.

How to Get Rid of a Lower Back Spasm: Immediate Steps

Initially, rest is important:

So the first thing is that whenever you get a sudden spasm in the lower back, you do not have to do exercises at all; you actually have to rest so as to allow the spasm to relax.
I have observed that people go for exercises, different positions, but you do not have to do anything, just go for rest for about a day or two days or till this doesn’t get subsided completely. 
Let me tell you the reason behind this: why does a spasm develop in the low back? You must have felt that low back spam becomes very common in cold weather or in the rainy season when the humidity increases in the air. 
So, that is why we advise that whenever you have a spasm in your low back, you should not do exercises. If you do exercises, the contracted state will increase, due to which, instead of reducing the pain, the pain and spasm will increase.
So you go to bed to rest. Along with bed rest, you should also adopt some other measures.

Ointment + Heat Method for Immediate Relief:

My most useful tip, which I usually give to my patients, is you apply some good pain gel or ointment in the low back, rub it well and then leave it for 10 minutes.
In 10 minutes, the body will absorb the ointment, the muscles will absorb it, and after that, apply hot treatment over there.

There are different ways of applying heat treatment. Let me mention them here; you can choose any of the modes at your convenience.

The approach of rest and heat is also strongly supported by ancient medical systems like Ayurveda. Research analyzing lower back spasm through the eyes of Ayurveda identifies this condition as Katigraha—a disorder caused by an imbalance of the Vata dosha, characterized by pain (Shoola) and stiffness (Stabdhata).

The primary Ayurvedic treatments for balancing Vata and relieving Katigraha directly complement what we’ve discussed:

  • Abhyanga (Therapeutic Oil Massage): Similar to the pain ointment we recommended, medicated oil massage helps pacify Vata and relax muscles1.
  • Swedana (Heat Therapy): This is the cornerstone of Ayurvedic treatment for stiffness, perfectly aligning with the hot fomentation, hot water bag, and infrared lamp methods we just covered1.

Different ways of giving heat therapy

1) Hot fomentation

Hot fomentation is perhaps the most accessible way of all, as it can be done with things available at home.

  1. First, take a bowl of water and heat it over the gas stove to a temperature of around 45 degrees.
  2. Add a tablespoon of salt to it and take a thick towel.
  3. Now, dip the towel in it and squeeze it, then apply it over the painful areas on your lower back.
  4. Apply hot fomentation twice or thrice daily.

This is the best and most effective method because, in hot fomentation, the steam quickly absorbs into the muscles. Do this procedure thill the water comes to a normal temperature.

2) Hot water bag

There is another way of applying heat treatment, the use of a hot water bag made of rubber. It is also termed a hot water bottle. This is a simple and hassle-free method in which you need to heat the water and pour it into the bag. Then simply apply it to your back.

3) Infrared lamp

There is a third way which I personally prefer over the other, it infrared lamp. So, it is a simple lamp with a special infrared bulb installed in it that emits infrared rays.

A red colour warm light comes out of it, so that you can apply it after applying the ointment. You just have to plug it in, and light will come out of it, and you can take it around your low back. If you are not able to do it alone, then take the help of your partner.

So this is my preferred method because it is also very easy to use. The other methods also work, but it is not always possible to do it, it is kind of inconvenient if you are living alone. Learn here how to use an infrared lamp?

When to See a Physiotherapist or Ayurvedic Practitioner

While the methods above work fine for acute, sudden spasms, persistent or severe pain requires professional intervention. The same research paper outlines more intensive therapies used by professionals when home care is insufficient:

  • Physiotherapy Treatments: This may include ultrasound therapy or Interferential Therapy (IFT) to deeply reduce inflammation and muscle spasm.
  • Advanced Ayurvedic Treatments: A qualified practitioner might recommend procedures like Basti (medicated enema), which is considered a primary treatment for Vata disorders, or Agnikarma, a precise heat therapy, for chronic cases.

If your pain is severe, you don’t feel any improvement after 2-3 days of rest, or if it is accompanied by numbness, tingling, or weakness in your legs, it’s essential to consult a healthcare professional—whether a physiotherapist or an Ayurvedic doctor—for a proper diagnosis and tailored treatment plan.
